CORE

Community, Outreach, Recovery and Engagement

One-to-one, community-based support for people in the Bradford District with severe mental illness (SMI) to help set goals. This partnership service exists to support people who cannot currently access statutory services or are transitioning between statutory and community support.

 What does the support look like?

Referral

A professional can refer you to CORE. This could be a mental health practitioner based at your GP Practice, Community Mental Health Team or someone from the Assessment Team.

Initial contact

A CORE support worker will contact you within 5 working days to start the process.

First appointment

Your first appointment will take place after a further 10 working days. During this meeting, the support worker will understand your needs and set goals to guide your care.

Support sessions

You will have up to 12 sessions of support, with reviews at the 6th and 12th sessions to check progress and discuss your next steps. If more support is needed, we’ll plan this with you.
